Georgia Form MV-9D (Disabled Person's Parking Affidavit): Applying for a Disability Parking Placard in Gwinnett County
September 30, 2026
If you have a qualifying medical condition that makes walking long distances difficult, a Georgia disabled parking placard or license plate can make everyday errands and appointments much more accessible. The official application for this accommodation is the Georgia Form MV-9D — Disabled Person's Parking Affidavit, also known as the Person with Disability Parking Placard/License Plate Application.
What Is Form MV-9D Used For?
Form MV-9D is the official Georgia Department of Revenue application used to obtain either:
- A temporary disabled parking placard (valid up to 180 days, renewable)
- A permanent disabled parking placard (valid for the applicant's lifetime)
- A disabled person's license plate (permanent; replaces the placard on the registered vehicle; costs $20)
The form allows individuals with qualifying disabilities — including mobility impairments, lung disease, cardiac conditions, and vision impairments — to lawfully park in designated accessible parking spaces throughout Georgia.
Why Does Georgia Require Notarization?
Notarization is required on Form MV-9D because the form serves as a sworn affidavit. The healthcare practitioner certifying the disability must sign Section C of the form in the presence of a Georgia notary public. The notary verifies the doctor's identity and administers the oath, affirming that the medical information provided is truthful and accurate under penalty of perjury.
The following licensed healthcare providers are authorized to certify your disability on the MV-9D:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Podiatrist
- Optometrist
- Licensed Chiropractor
The healthcare provider's signature must be notarized, and this requirement helps prevent fraud in the disability parking system, ensuring that accessible spaces remain available for those who genuinely need them. This requirement is established under O.C.G.A. § 40-2-74.1.
